Condition of original material: The file contains an incorrect date and an incorrect number of pages on the title page. The pages are arranged out of numerical order (for example: fol. 13, 25, 26, 14, 27, 28, 15, 29, etc.). The printed forms are in a standard format with headings, filled out by hand with black, blue, and purple ink. Some forms are typed (fol. 60, 78). The forms were printed in 1933 by the Revision Union of Ukrainian Cooperatives (fol. 1-78). Other forms were printed in 1936 (fol. 79-84). Strikethroughs are made with black ink. Additions are in red, gray pencil, and pink ink. The pages have yellowed and are covered in yellow stains, with ink stains. All pages are pasted on the left side with white paper. Small tears in the forms are patched with transparent paper (on the right side, at the bottom, lower right corner, and along the top horizontally). The file contains the oval stamp of the Women's Publishing Cooperative "Union of Ukrainian Women" in Lviv (two stamps per page on fol. 1-23, 25, 27-38, 40-72, 74-77, 79; one stamp per page on fol. 24, 26, 39, 73, 78, 80).
