Condition of original material: Manuscript. The text is written in black ink. Separate entries, underlining, crossing out, and markings are made in pencil (gray), red, blue, and black ink. The form is printed and filled in with black ink. The sheets are yellowed, contain yellow, brown, and ink stains. The sheets are glued with transparent and white paper on the left side. Individual folios are glued with transparent paper on the top, bottom, and right sides of the sheet. In individual sheets, part of the sheet is torn off: upper right corner (fol. 33); lower right corner (folios 27, 33); lower left corner (fol. 124); right side (folios 6, 43, 102); top (folios 13, 20, 44, 76, 99, 105); bottom (fol. 82); cut out in the middle of the sheet and part of the sheet on the right side (fol. 98).
