Size and dimensions of original material: Fol. 1-6: height 34 cm, width 21 cm.
Condition of original material: Printed forms are filled out by hand. Texts are written in blue pencil. The blue ink of the document has faded. Oxidized paper are visible on individual sheets. Folios are glued on the inside with white paper.
