На документах збереглися відбитки прямокутного штемпеля кооперативу "Лікарська самопоміч" у м. Львів (арк. 8 зв.).

Size and dimensions of original material: Fol. 1-8: height 34 cm, width 21 cm; fol. 9-10: height 33.3 cm, width 21 cm.
Condition of original material: Printed forms are filled out by hand. The texts are written in black pencil. In blue - signatures, marks, individual words; signatures in green. There are numerous yellow spots. The sheets are glued on the left side with white paper.
