На документах збереглися відбитки прямокутних штемпелів кооперативу "Лікарська самопоміч" у м. Львів (арк. 1, 8 зв. 9, 10 зв.).

Condition of original material: The printed forms are filled in by hand. Fol. 1: part of the paper with text printed on a typewriter is glued. The texts are written in black pencil. The sheets are glued on the left side with white paper.
