Condition of original material: The printed forms are filled in in black and blue pencil, with some entries in green. Manuscripts. The blue ink of the document has faded. Fol. 21, 23, 26: there are yellow spots of oxidised paper visible on individual sheets. Fol. 5: the top of the sheet is torn.
