Condition of original material: The forms printed in the printing house are filled in by hand with a blue pencil; the marks are made in green. The text is typed on a typewriter. There are yellow spots of ocm, width idized paper are visible on individual sheets. The condition of the inscriptions is legible. Sheet 7 is torn at the bottom right side.
