Condition of original material: Fol.. 1-10, 11-20 v.: the text is printed on a typewriter. Handwritten corrections are made in black ink. Weak saturation of letters. The folios are partially damaged and are glued around the perimeter with transparent tape. Fol. 22-29: printed application forms filled in by hand.
